This version of Dead or Alive will run on a heavily modified version of the engine used in Dead or Alive 4, allowing for new inclusions such as self-shadowing. The Marine Race mini-game, however, will use a completely new engine, although it looks very similar to the rest of the game. There will be a cross-connection between DOAX2 and Dead or Alive 4, with saves from one game having a substantial impact on the other.

The story takes place on New Zack Island, with the opening cinema showing how the resort came into Zack's possession and the lie he uses to deceive the girls once again.

The volleyball mechanics will be enhanced for better control, with an emphasis on spiking and blocking to improve strategies and techniques. This was introduced as the previous game was thought to be "too easy" and relied quite heavily on feints, making it less challenging and fun.
There will be three separate hotels: Gemstone Suite, Seabreeze Cottage and Moonlight Reef.

Creator Tomonobu Itagaki is against ever adding activities like trampoline games and wet T-shirt contests to the series, as he feels they would come off as vulgar and "show the women in a negative light".

Each breast has its own physics. This allows each breast to behave differently. This has been criticized, however, for making the breasts move too much (they often continue moving for an unnaturally long time after a character has stopped moving), and was parodied in a comic by

The game will be playable on Xbox Live, with both the Beach Volleyball (one-on-one) and Marine Race games announced as viable for online play.

As in the original game, the beginning and ending to DOAX2 will feature a pre-rendered clip.

Players will be able to use a Cameraman Mode. With this feature, players can use the camera and rotate it 360 degrees, allowing for easier control to photograph their favorite scenes, girls, or poses. Pictures will not be able to be officially traded or shared over Xbox Live.

Featured Mini-Games
The game will include 7 Mini-Games (excluding casino games):
* Marine Race, in which the girls race on jet skis, is supposed to be the new headlining game. It is apparently fairly similar in both look and feel to the Wave Race series. Players earn nitro boosts (which give them a substantial, but temporary, increase in speed) by navigating through five consecutive buoy gates. Players can earn in-game currency by winning the race and by doing tricks. Players can choose to lose the race by a long shot and simply navigate the track indefinitely, pulling off tricks to earn cash. This mode will be playable online via Xbox Live.

* The former "main event," Beach Volleyball, will return. The two-on-two gameplay is mostly unchanged, though blocking shots will be more common. The player will choose a computer-controlled teammate to compete against two other computer-controlled opponents. In the online mode, two players will compete against each other, each having a computer-controlled partner. The camera system has been slightly modified to address the criticism that the one in the original game moved too much.

* Beach Flags is one of the simplest new games, but quite difficult. It requires the player repeatedly press a button as fast as possible to reach a flag at the end of the beach before their opponent does. To successfully grab the flag, the player must not only be fast but also correctly judge the distance to dive. If the player is too slow, falls short, or over shoots the flag, they lose. False starts also result in instant loss.

* The Hopping minigame from the previous title will return in a more competitive and slightly harder fashion. Now called Pool Race, players will race another girl, hopping across multi-colored floating pads. To successfully hop to the next pad, the player must press the controller button of the corresponding color. Pressing the button quickly executes a short jump, and holding the button for a moment executes a longer jump. Bonus points can be earned for making particularly well-timed jumps.

* Butt Battle is a game in which the player attempts to knock their opponent off a shared floating pad before the same can be done to them. It involves feigns, dodges, normal attacks, and strong attacks, which the girls carry out using only their buttocks.

* Water Slide is the last game, though details about it are mostly unknown. The basic gameplay revolves around keeping the player character from falling out of the slide as it twists and turns. Pushing forward on the left joystick speeds your character up, and pulling back slows them down. Left and right are used to prevent your character from falling out of the slide.

* In Tug-of-War the player attempts to make their opponent fall off of their distant floating pad. Player victory can be achieved both by pulling their opponent into the water or by tricking them, using a feign move, into accidentally throwing themselves in.
